Assist patients in navigating hospital programmes and digital health platforms, contributing to the smooth delivery of patient care.

🎯 About the Role

This role involves supporting patients in understanding and utilizing hospital programmes and digital health tools. You will engage with patients to ensure their adherence to programme workflows and collect valuable feedback.

🛠️ Key Responsibilities

• 🤝 Assist with patient onboarding into Smart Triage and TEMPO programmes.
• 📱 Guide patients in navigating digital platforms such as FormSG or MyChart.
• 📞 Engage patients through telephone calls or face-to-face interactions to address queries.
• 📝 Administer feedback surveys and collect, compile, and maintain patient feedback records.
• 📁 Maintain accurate records of patient onboarding and engagement activities.

✅ Requirements & Skills

• 🧐 Meticulous, organised and attentive to detail.
• 🗣️ Strong verbal and written communication skills.
• 💬 Comfortable engaging patients by telephone and face to face.
• 💻 Able to support patients with digital platforms.
• 🔐 Able to maintain accurate records and handle data responsibly.
